Instead of driving to the researchers’ location every few weeks, participants can participate in the study from their homes.

They typically do this by wearing a smartwatch that tracks their heart rate or sleep, having a courier pick up samples from their porch, or by logging data into a phone app made by the researchers. Similarly, consent forms are often signed electronically, on a tablet or phone, rather than on paper in a waiting room.

The FDA finalized guidance on this model in September 2024, and the agency makes it clear that expectations for data quality and safety monitoring are the same as for remote trials as they are for in-person ones.

A Day in the Life of a Remote Participant

How a day during a remote trial might look will depend heavily on the exact study. However, they typically include having to wear blood pressure cuffs, glucose monitors, and heart rate sensors, and other wearable gadgets.

Often, these send readings straight to the study team with an app that you have to sync with the tools. You might also have to log your symptoms and data on the app.

And instead of an in-person consultation, researchers might have a scheduled video visit with a study nurse. Video calls are also how patients and researchers communicate if something looks off.

Sometimes, if there’s a need for in-person testing like a blood draw, a mobile phlebotomist can come to your home instead of you having to leave the house.

Does Remote Participation Actually Work?

Yes, it does—and that’s why remote studies have become more popular.

A survey conducted by digital healthcare solutions provider Vivalink found that 90% of its over 200 American participants were open to participating in remote clinical trials, and 52% were “very likely” to join if they could do so from home. Only 34% said the same about in-person trials.

As Vivalink’s CEO, Jiang Li says, it’s now “more than just a convenience; they’re a prerequisite for many people considering participation.”

However, being willing to join one and actually staying in a study’s entire duration are two different conversations. A large decentralized trial that used a wearable to help detect COVID-19 in more than 17,000 participants says that it does, with roughly 80% of participants still being active after six months, and about 68% stayed enrolled to the end.

Common Concerns About Remote Trials

Still, remote trials come with their own concerns.

First and foremost, reliability: what happens if a wearable stops syncing or an app crashes during an important moment? In an on-site study, the coordinator will just be down the hall to fix things.

For remote trials, participants will have to call the team—and the team needs to go to the participant’s place if the issue can’t be fixed remotely. However, good study teams should make tech support easy to reach and explain.

Another concern is privacy. Trials carry sensitive data, and the apps used run on your own phone and home Wi-Fi, which can create additional risks if those devices or networks are not properly secured.

This is even more of a concern when participants are travelling and need to use public Wi-Fi. In such cases, data privacy tools like a VPN, such as the one here, are a simple way to protect one’s data when away from home.

Lastly, access. Not everyone has a reliable smartphone, high-speed home internet, or a quiet space for video visits, which raises questions about who gets left out of a study built around remote tools.

The Bottom Line

Instead of repeat trips to research sites, decentralized remote clinical trials are becoming more popular. This allows for both more flexibility and a greater willingness from patients to participate in these studies.

Federal guidance backs the approach, and early data suggests people stay enrolled when the technology is simple and support is close at hand. However, keep in mind that there might be issues with reliability, privacy, and access before you sign up for one. It’s crucial to understand these things before you let a study into your daily life.

Image by KOS Chiropractic Integrative Health from Pexels

The editorial staff of Medical News Bulletin had no role in the preparation of this post.
